Annual Physical - 3 Easy Steps (must submit all 3 documents)

Under the ESHCA Health Plan your annual physical is covered at a $0 co-pay. Having members use their preventative appointments (annual physicals, dental and vision checkups), reduces the risk of future high-cost claims. The member and the member-spouse can receive $200 for submitting the following paperwork:

1.  Explanation of Benefits (sample)

   • a) What am I looking for? Page 2 of the EOB should state:

     • "Description: Est Pt Prevent Healthy 18 to 39 Yrs" (or your age range)​

   • b) Where can I find my EOB? Gia or mail

     • GIA (the MVP App) ​

       • Locate the EOB under the "Claims" tab, take a "screen shot" of the EOB with the correct "Description" (above), attach to an email and send to Patrick, paper copies can be mailed or faxed to the ESHCA office, attn: Patrick Flaherty, Wellness Coordinator.​

     • Mail

       • You should receive a paper copy in the mail a few weeks after your appointment.​​
